Annual Survey Terms & Conditions

1. One Entry Per Person: Each individual is permitted to submit only one response to the annual survey.

2. Completion Required: All survey questions must be fully completed in order to qualify for the voucher.

3. Non-Transferable & No Cash Exchange: The voucher is non-transferable, cannot be exchanged for cash, or any other alternatives.

4. Voucher Validity: The voucher is valid for a fixed period as stated. Extensions to the validity period will not be granted under any circumstances.

5. Mailing of Vouchers:
   a. All vouchers will be mailed via SingPost to the postal address provided in the survey response.
   b. No changes to the delivery address will be accepted after submission of the survey.

6. Mailing Risks: The Co-operative will not be held responsible for any loss, delay, or damage to the voucher once it has been mailed.

7. Right to Amend: The Co-operative reserves the right to amend these terms and conditions without prior notice.
