Members may:
i) request the change online: Monthly Deduction Change Form
ii) download the Change in Monthly Deduction Form from our website and thereafter to email/fax/mail in the completed form to us for processing; or
iii) drop by our office.

This change can only take effect at least 2 months from the month of change request.
