Please note:

  1. Signature is required for all application forms.
  2. Please print and sign on the hard copy after clicking on the ‘Submit’ button.
  3. A copy of the completed Individual Tax Residency Self-Certification Form has to be submitted together with the Membership Application Form. A copy may be printed here.
  4. All membership applications are to be submitted by the applicant personally to our office
    together with the required documents.
